1. Go to our procedures and select what procedure is desired. If moving forward and would like special pricing, reserve your procedure or consult and date by filling out the form below. We will need a $1,000.00 deposit for this with credit card via PayPal. We will contact you once we receive the form back for this. If you would like to schedule a consultation only please fill out the form below, reserve your month and date and use PayPal to pay the $150.00 to reserve an office consultation using our State of the Art Digital Imaging.

Instate, local or out of state and international patient's we now have our online Reservations for Consultations and Surgeries. This service we offer makes it simple to schedule a Phone Consultation or Schedule an appointment to come into our office for your consultation.

We provide the latest in Digital Imaging that is by far the greatest technology for you and the doctor to get the results that you desire.

This will give the patient the advantage of actually seeing their surgical result with the doctor prior to the surgery, allowing the patient their input on the desired end result. See our Photo Gallery with before and after photos.

We have our concierge that will book the hotel and airline reservations for you at the best prices available to us as well as schedule your Consultation by Phone or by coming into our offices.

We provide online surgery schedule with the month and day you would like to come in and offer a Special pricing for couples or multiple Surgeries.

Our Facility is complete with a highly experienced R. N. Aesthetician who will provide skin care packages for your needs. In office Consultations are a fee of $150.00 complete with Digital Imaging.

Phone consultations with the doctor and his facility director / consultant are complimentary. Please feel free to email us your photos. We will review them and provide a surgical plan and fee quote.

You will meet with the Head Facility Director/Consultant Deborah Canter and Dr Richard Ellenbogen at our offices located in West Hollywood/Beverly Hills location. If you would like to schedule a consultation and Digital Imaging and plan to then have surgery, please select the month and day you would like to come in. The following will be needed:

• On-line or phone consultation with Our Facility Consultant Deborah Canter
• On-line or phone consultation with our Doctor
• If you want to schedule a trip to our office and Consultation and Schedule Surgery a deposit of $1,000.00 will be needed (credit card)

All deposits for scheduling surgery will be a fee of $1,000.00. CANCELLATIONS FOR REASONS OTHER THEN MEDICAL NECESSITY WILL RESULT IN A CHARGE AMOUNTING TO 25% OF THE OF THE SURGEONS FEE, IF MADE AT LEAST TWO WEEKS FROM YOUR SCHEDULED SURGERY DATE. CANCELLATIONS FOR REASONS OTHER THAN MEDICAL NECESSITY MADE LESS THAN TWO WEEKS FROM YOUR SCHEDULED SURGERY DATE WILL RESULT IN A CHARGE AMOUNTING TO 60% OF THE SURGEONS FEE. THIS IS NON-NEGOTIABLE.

We offer financing programs as well as discounts for surgeries scheduled on the day of your consultation. The fee is a discounted depending on procedure(s).
